<p>A report by South Africa's Civil Aviation Authority has identified an "uninsured" penguin as the cause of a helicopter crash after it slipped and hit the pilot's controls. The incident occurred during an aerial survey on Bird Island. Although the helicopter sustained considerable damage, no one on board, including the penguin, was injured.
