An Israeli army investigation has concluded that there were "professional failings" following the killing of 15 members of Gaza's emergency services in March. The incident was the result of operational misunderstandings, with soldiers mistaking ambulances for threats. A commander will be dismissed and the military prosecutor-general will consider possible criminal charges. The army admits the mistakes occurred in a "complex combat zone".
