As artificial intelligence (AI) becomes increasingly integrated into business operations, the risks are not just dramatic errors, but also silent failures on a large scale.

Modern AI systems are so complex that developers cannot predict how they will behave in real-world environments. These silent failures can cause operational problems and compliance risks without being immediately evident. For example, an AI system in a beverage company generated excess production due to an incorrect label, demonstrating how AI can act logically but not intelligently. Additionally, customer service AI systems can create unexpected incentives, such as approving refunds outside the company's policy. Experts emphasize the need for implementing rapid intervention mechanisms, such as a "kill switch," to manage unpredictable behaviors. Governance is also essential, and organizations must build operational safety measures, including human oversight of the system's behavior.

Despite the risks, companies continue to adopt AI for competitive reasons, which can lead to systemic failures. The future of AI will depend not only on better algorithms but also on smarter governance.
