The 300-page report, the result of a two-year investigation, points out that OceanGate, the company responsible, had serious deficiencies in design, certification and maintenance. The commission pointed to a "toxic organizational culture" and insufficient regulation, saying OceanGate evaded government scrutiny and failed to fix Titan's known problems. The incident led to the deaths of five people, including CEO Stockton Rush.
