The US Coast Guard has concluded that OceanGate is primarily responsible for the Titan submersible's implosion, which killed five people.

The Coast Guard report points out that OceanGate ignored safety, testing and maintenance protocols, which led to the June 18, 2023 accident. The submersible Titan, exploring the wreck of the Titanic, was destroyed by a catastrophic implosion, killing all five passengers. The investigation revealed company negligence and a toxic work environment that discouraged employees from reporting safety issues.
